In a 5-quart crock pot, stir together the heavy whipping cream, milk, sweetened condensed milk, vanilla extract, and milk chocolate chips. For a doubled recipe, use a 6-7 quart slow cooker.

Cover the crock pot and cook on low for 2 hours. Stir occasionally with a whisk to ensure the mixture is well combined and the chocolate chips are melting.

Once the mixture is heated through and the chocolate is fully melted, switch the slow cooker to the 'warm' setting. Stir again before serving. If melting the chocolate takes longer, turn the slow cooker to high temporarily.

Serve the hot chocolate in individual cups, garnished with whipped cream or marshmallows as desired.
