In a medium saucepan, combine the whole milk and heavy cream. Heat over medium heat until just simmering, stirring occasionally to prevent burning.

Reduce the heat to low and add the finely chopped dark chocolate, stirring continuously until completely melted and smooth.

Add the sugar, vanilla extract, and a pinch of sea salt to the chocolate milk mixture. Stir until well combined and the mixture is thick and creamy.

Taste and adjust the sweetness by adding more sugar if desired. Continue to heat on low for another minute without boiling.
